Coleman, III to perform services to develop a natural health improvement program for me in order to assist improving my overall health & NOT for the treatment “cure” of any disease. I understand that the services rendered are methods of balancing my physical, emotional & nutritional needs.  I will continue to see my current medical doctors & prescribed medication should not be altered without first consulting my Doctor. Nothing said, done, typed, printed, reproduced by Get Fit With Mint, LLC.,Armintie J. Coleman, III is intended to diagnose, prescribe, treat or take the place of a licensed physician. I understand that Get Fit With Mint, LLC.,Armintie J. Coleman, III is not liable for my healthcare. I hereby RELEASE, WAIVE, DISCHARGE, & COVENANT NOT TO SUE Get Fit With Mint, LLC. Armintie J. Coleman, III & its Owners, Volunteers& all other person(s,) affiliates, entity(ies) from any & all liability, claims, demands, actions & causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in such activity, while in  or upon the premises where the activities are being conducted, REGARDLESS OF WHETHER SUCH LOSS IS CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise & regardless of whether such liability arises in tort, contract, strict liability, or otherwise, to the fullest extent allowed by law. This grant of use includes but not limited to internet & emails.  My images are property of Armintie J. Coleman, III & Get Fit With Mint, LLC. This release extends to all formats. This release is indefinite. I waive rights to royalties. I understand & agree.
